Welcome to Cleveland Community Home and Support Services, Incorporated (CCHSSI). We provide experienced and companssionate Targeted Case Management Services.
Our team of diverse professionals work to meet whatever needs and preferences you may have. We specialize in providing Targeted Case Management to individuals with Developmental Disabilities, Dual Diagnoses, and Traumatic Brain Injuries.
CCHSSI incorporated in July 29th 2004. Prior to that CCHSSI was providing Case Management Services under Cleveland Vocational Industries, Inc. which has been providing human services since 1984.
CARF has accredited CCHSSI for its Community Services Coordination.
